- Optimizes Patient Care: Ensures that the services provided by the healthcare team are delivered efficiently and cost-effectively, while striving for the best possible patient outcomes.
- Resource Management: Collaborates with physicians and the healthcare team to promote judicious use of healthcare resources.
- Care Admission: Guarantees that patients are admitted to the suitable level of care and that services are rendered promptly, adhering to established criteria and regulatory standards.
- Discharge Planning: Works closely with patients, caregivers, and the healthcare team to evaluate discharge requirements and develops a safe and appropriate discharge strategy.
Minimum Qualifications
Education: Must be a graduate of an accredited nursing program; a Bachelor of Science in Nursing (BSN) is preferred.
Experience: A minimum of 3-5 years of experience in acute care or community nursing is required.
Licensure and Certification: A current registered nurse license in Massachusetts is mandatory; Certified Case Manager (CCM) certification is preferred.
