Position Overview: The Senior CCBHC Case Manager plays a crucial role in providing comprehensive case management and crisis intervention services. This position is designed to facilitate the coordination of treatment services in alignment with workforce initiatives, ensuring clients have access to transitional support services while addressing various challenges such as sanctions and barriers to employment.


Key Responsibilities:

  • Lead and facilitate support groups focusing on essential life skills.
  • Act as an advocate for clients to secure necessary services.
  • Collaborate with relevant agencies and systems (e.g., DSS, probation, courts) to assist clients in fulfilling all mandates.
  • Provide ongoing case management addressing needs related to housing, transportation, childcare, nutrition, and other essential services.
  • Administer toxicology tests as required.
  • Maintain effective communication with direct service and vocational service providers.
  • Submit weekly schedules and monitor the effective use of time.
  • Participate in staff and community meetings as appropriate.
  • Deliver crisis intervention and group work services, providing backup coverage when necessary.
  • Ensure proficiency in computer skills relevant to job performance.
  • Uphold quality care standards in accordance with the agency's Medicaid Compliance Plan and accountability systems.
  • Report any compliance violations to the compliance officer or supervisory structure as per agency policy.
