Position Overview:

The Case Management Supervisor plays a crucial role in overseeing the operations of the Emergency & Transitional Housing Program (ETH), which provides essential transitional housing and support services for individuals reintegrating into the community after involvement with the justice system. This position involves managing a team of case managers and ensuring the delivery of high-quality services that address the unique needs of participants.

Key Responsibilities:
  1. Program Oversight:
    • Manage daily operations of case management and housing assistance services.
    • Collaborate with program leadership to develop and implement effective policies and procedures.
  2. Participant Support:
    • Facilitate access to vital support services addressing health, employment, and housing needs.
    • Foster a supportive environment that promotes participant well-being.
    • Ensure all participants have comprehensive housing and service plans.
    • Address participant grievances and conduct necessary interventions.
    • Maintain a caseload of high-need clients as required.
  3. Care Coordination:
    • Assist in coordinating care with internal and external service providers to enhance program outcomes.
  4. Staff Management:
    • Supervise a team of 3-4 case managers and provide guidance to support staff.
    • Conduct performance evaluations and participate in recruitment and onboarding processes.
  5. Training and Development:
    • Lead training initiatives for the case management team to ensure effective service delivery.
    • Support the creation of ongoing training programs.
  6. Quality Assurance:
    • Ensure compliance with internal and external service delivery standards.
    • Conduct regular quality assurance audits and utilize findings for continuous improvement.
    • Maintain accurate and up-to-date client records.
  7. Advocacy and Engagement:
    • Promote engagement in advocacy efforts aligned with the organization's mission.
    • Exemplify the core values of the organization in all interactions.
Qualifications:
  • Bachelor's degree in social services, psychology, social work, or a related field is preferred.
  • Master's degree with active NYS LMSW, LCSW, or LMHC license is preferred.
  • Minimum of 3 years of experience in case management or direct client services.
  • At least 1 year of supervisory experience.
  • Experience working with individuals affected by the criminal justice system or co-occurring disorders.
  • Strong communication and documentation skills.
  • Proficiency in Microsoft Office Suite, particularly Excel, and experience with EMR systems.
Additional Competencies:
  • Preferred bilingual proficiency in Spanish.
  • Commitment to serving homeless individuals impacted by the justice system.
  • Ability to maintain a strengths-based, harm reduction approach.
  • Flexibility to work evening hours as needed.
