Transition Program Coordinator

* Implement Love146's Survivor Care program's Transitional Services programming services with excellence.
* Identify individualized Care Plan goals and measurable objectives for program participants in English and Spanish.
* Provide direct services to program participants in English and Spanish to help them achieve their Care Plan goals.
* Liaise between program participants, educational institutions, employers, housing resources, childcare resources, and other government and community-based agencies to support program participants in achieving their Care Plan goals.
* Track participant progress and modify services and goals as needed.Required Skills & Qualifications:
* Bachelor's degree in a related field (e.g., human resources, social work, business, or education) preferred; or Associate's degree + 2-3 years of related work experience.
* Prior transitional services experience (e.g., workforce development, job placement, post-secondary education, and/or housing placement).
* Experience working directly with adolescents and young adults, preferably with high-risk populations.
* Spanish proficiency required; previous experience comprehending and communicating in Spanish in a professional setting highly preferred.
* Valid driver's license and clean driving record.Estimated Salary: $57,000 - $59,000 annually.Company Culture:
