As a Care Coordinator at AccentCare, you will play a vital role in ensuring our patients receive the highest quality care and support. You will work closely with our clinical teams to coordinate patient care, communicate with families and caregivers, and provide emotional support and guidance. Your compassion, empathy, and strong communication skills will make a real difference in the lives of our patients and their loved ones.

Qualifications:

  • Valid license as a Licensed Vocational Nurse or Registered Nurse
  • Minimum of one year experience as an LVN or RN in a medical setting, preferably home health
  • Knowledge of state and federal and accreditation requirements
  • Data entry skills, excellent verbal and written communication skills (especially telephone)
