Key Responsibilities:
- Collaborate with the healthcare team to develop and implement individualized care plans for residents.
- Accurately document resident conditions and nursing needs in a timely manner.
- Administer medications, execute treatments, and initiate physician orders as directed.
- Communicate effectively with families regarding changes in medications and resident status.
- Coordinate admissions, discharges, and transfers to ensure exceptional customer service.
Requirements:
- Graduate from an accredited nursing program.
- Current Licensed Practical Nurse license and other state-mandated licensure.
