Care Coordinator Job Description

At Crossroads Turning Points, we are seeking a dynamic and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing personalized care to patients in our program, developing skills to act as a counselor, and facilitating life skills groups.

Key Responsibilities:
  • Develop care planning and patient needs assessments
  • Facilitate life skills groups according to CTP policies and procedures
  • Act as a liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies
  • Document admissions, care management needs, discharge, and discharge planning
  • Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake
  • Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators
  • Assist treatment team with patient intake, transfer, and discharge assessments
  • Collaborate with the facility treatment team (medical, psychiatric, and SUD) to identify patients' needs
  • From available resources, assist patients in meeting their basic needs (i.e., clothing, medical, judicial, religious, etc.)
  • Coordinate services for treatment courts on behalf of CTP
Qualifications:
  • High School diploma or GED requires two years of clinical substance disorder experience
  • Bachelor's degree in a behavioral health-related field (preferred)
  • Obtain Certify Addiction Technician (CAT) certification within 18 months of the hire date
  • Obtain case management certification within 90 days of hire
  • Minimum of two years of experience working in substance misuse and mental health
  • Must have a good understanding of substance misuse, mental health, and treatment delivery systems
  • Advanced computer skills required
  • CPR and first-aid certification (the agency pays for initial training and renewals)
  • Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary
  • Valid driver's license and good driving record (must provide a current unofficial DMV driving record upon hire)
  • Able to communicate effectively in a multidimensional treatment program, including medical, psychiatric, and substance use related
