Job Summary

Crossroads Turning Points is seeking a dynamic Care Coordinator to join our Assessment team. As a key member of our team, you will be responsible for providing care coordination for patients in our program.

Key Responsibilities
  • Develop and implement care plans that meet the unique needs of each patient
  • Facilitate life skills groups and provide education on healthy lifestyle choices
  • Act as a liaison between hospitals, community health centers, and state and human service agencies to ensure seamless patient care
  • Document patient admissions, care management needs, discharge, and discharge planning
  • Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake
  • Organize patient bed dates, transitioning levels of care, and communicate with Medicaid Placement Coordinators
  • Assist the treatment team with patient intake, transfer, and discharge assessments
  • Collaborate with the facility treatment team to identify patient needs and develop effective care plans
Requirements
  • Associate degree in behavioral health or related field
  • Ability to obtain Certify Addiction Technician (CAT) certification within 18 months
  • Good understanding of substance use, mental health, and treatment delivery systems
  • Advanced computer skills
  • CPR and first-aid certification
  • Valid driver's license and good driving record
  • Ability to communicate effectively in a multidimensional treatment program
