Job Summary

UCHealth is seeking a skilled Nurse Coordinator to join our Transplant Care team. As a key member of our multidisciplinary team, you will provide exceptional patient care and coordination services to patients undergoing solid organ transplant. Your expertise and compassion will make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Coordinate patient care through collaboration with inpatient and outpatient multidisciplinary teams, pharmacies, and community providers.
  • Manage patient medications per policies and procedures.
  • Triage patient issues and facilitate coordination of patient care as directed by providers or per protocol.
Requirements
  • Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program.
  • State licensure as a Registered Nurse (RN).
  • 2 years of experience in a related field of specialty, preferably in transplant coordination.
