Care Coordinator

At Crossroads Turning Points, we are seeking a dynamic Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing care coordination for patients in our program, developing skills to act as a counselor, and facilitating life skills groups. You will also act as a liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies about patient services and needs.
Responsibilities include:
- Developing care planning and patient needs assessments
- Facilitating life skills groups
- Acting as a liaison and establishing networks and collaboration with hospitals, community health centers, and state and human service agencies
- Documenting admissions, care management needs, discharge, and discharge planning
- Administering case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake
- Organizing a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators
- Assisting treatment team with patient intake, transfer, and discharge assessments
- Collaborating with the facility treatment team (medical, psychiatric, and SUD) to identify patients' needs
- From available resources, assisting patients in meeting their basic needs (i.e., clothing, medical, judicial, religious, etc.)
- Coordinating services for treatment courts on behalf of CTP
Qualifications:
- High School diploma or GED requires two years of clinical substance disorder experience
- Bachelor's degree in a behavioral health-related field (preferred)
- Obtain Certify Addiction Technician (CAT) certification within 18 months of the hire date
- Obtain case management certification within 90 days of hire
- Minimum of two years of experience working in substance misuse and mental health
- Must have a good understanding of substance misuse, mental health, and treatment delivery systems
- Advanced computer skills required
- CPR and first-aid certification (the agency pays for initial training and renewals)
- Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary
- Valid driver's license and good driving record (must provide a current unofficial DMV driving record upon hire)
- Able to communicate effectively in a multidimensional treatment program, including medical, psychiatric, and substance use related
