Care Coordinator Job Description

At Crossroads Turning Points, we are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing personalized care to patients in our program.

Key Responsibilities:
  • Develop and implement care plans that meet the unique needs of each patient.
  • Conduct thorough patient assessments to identify areas of need and develop strategies for improvement.
  • Collaborate with treatment teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes to inform treatment decisions.
  • Provide education and support to patients and their families to promote healthy behaviors and lifestyle choices.
Requirements:
  • Associate degree in behavioral health or related field.
  • Ability to obtain Certify Addiction Technician (CAT) certification within 18 months.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Valid driver's license and good driving record.
