About the Role

We are seeking a highly skilled Client Care Specialist to join our team at Homes for the Homeless. As a key member of our Family Services department, you will play a vital role in providing mental health and related services for homeless families.

Key Responsibilities
  • Comprehensive Assessment: Conduct thorough bio-psychosocial assessments with each family to understand their strengths and service needs.
  • Service Coordination: Monitor, lead, and coordinate social service and counseling support, including timely interventions.
  • Effective Service Delivery: Ensure focus on effective social services delivery to clients, incorporating models and best practices such as trauma-informed and family-centered care.
  • Progress Monitoring: Monitor and document client progress toward service plan goals.
  • Support and Referral: Ensure families receive full support in shelter and refer them for counseling or other services when necessary.
  • Collaboration and Training: Work alongside the Case Manager and support staff when dealing with challenging clients, and assist with unit inspections and training sessions.
  • Linkages and Permanency: Improve linkages to mental health, education, and community-based services, and strengthen overall permanency outcomes for families with children in shelter.
  • Record Keeping and Evaluation: Manage record keeping, data collection, and evaluation on the effectiveness of services for families.
Requirements
  • Education: Master's degree in Social Work or Mental Health Counseling.
  • Licensure: LMSW (Licensed Master Social Worker) or LMHC (Licensed Master Health Counselor) in the state of New York.
  • Experience: Familiarity with substance use disorders and addiction treatment services, domestic violence, child abuse and maltreatment, trauma, and mental health conditions.
  • Skills: Sensitivity and awareness of working with homeless and vulnerable populations, and bilingual skills a plus.
