At CAMBA, we are seeking a highly motivated and compassionate Case Manager to join our team at The Landing Family Shelter. As a Case Manager, you will play a critical role in providing comprehensive case management services to families in a transitional residence program.

Job Summary:

The Case Manager will work closely with clients to establish and maintain professional relationships, engage them in case management services, and provide support to help them achieve their goals. This includes conducting initial intake and psycho-social assessments, creating and maintaining client files, and referring clients to permanent housing opportunities and community resources.

Key Responsibilities:
  • Establish and maintain professional relationships with clients and client confidentiality
  • Conduct initial intake and psycho-social assessments of clients and clients' families' needs
  • Create and maintain client files, including inputting client data and client progress information into the CARES database
  • Refer clients to permanent housing opportunities and community resources
  • Assist clients in completing applications for benefits and entitlements
  • Monitor clients' progress toward their goals and document via progress notes
  • Act as client liaison/client advocate with outside organizations
Requirements:
  • Bachelor's degree (e.g., B.A., B.S.W.) and/or equivalent experience
  • Ability to maintain clearances throughout the duration of employment
  • Bi-lingual preferred
