About the Role

We are seeking a highly skilled and compassionate Client Care Coordinator to provide mental health and related services for homeless families at Homes for the Homeless.

Key Responsibilities:

  • Conduct comprehensive bio-psychosocial assessments with each family to understand their strengths and service needs.
  • Monitor, lead, and coordinate social service and counseling support, including timely interventions.
  • Ensure effective social services delivery to clients, incorporating models and best practices such as trauma-informed and family-centered care.
  • Monitor and document client progress toward service plan goals.
  • Support families in receiving full support in shelter and refer them for counseling or other services when necessary.
  • Collaborate with the Case Manager and supporting staff to address challenging clients.
  • Improve linkages to mental health, education, and community-based services.
  • Strengthen permanency outcomes for families with children in shelter.
  • Manage record keeping, data collection, and evaluation on the effectiveness of services for families.
  • Support the Family Services department by conducting trainings and promoting best practices.

Homes for the Homeless operates shelters for families experiencing homelessness in NYC. Our family residences provide more than just a place to sleep, offering an array of onsite programs that support families in achieving their goals.

Qualifications
  • Master's degree in Social Work or Mental Health Counseling.
  • Licensed Master Social Worker (LMSW) or Licensed Master Health Counselor (LMHC) in the state of New York.
  • Familiarity with substance use disorders, addiction treatment services, domestic violence, child abuse and maltreatment, trauma, and mental health conditions.
  • Sensitivity and awareness of working with homeless and vulnerable populations.
  • Bilingual skills are a plus.
