Job Summary

We are seeking a Client Care Coordinator to join our team at The Landing Family Shelter. The successful candidate will provide support and advocacy to families in need, helping them to achieve their goals and improve their overall well-being.

Key Responsibilities
  • Assist clients in completing intake applications and screenings
  • Create and maintain client files and make file copies
  • Conduct initial risk assessments and prepare biopsychosocial evaluations
  • Provide instant response services and advocate on behalf of clients
  • Collaborate with clients and case managers to develop Independent Living Plans
  • Identify community resources and refer clients to appropriate services
  • Follow up with clients and referral organizations to ensure progress
  • Consult with internal and external stakeholders to determine causes of client problems and develop solutions
Requirements
  • Licensed Master Social Worker (LMSW) required within one year of hire
  • State Central Registry (SCR) clearance, The Register of Substantiated Category One Cases of Abuse or Neglect (SEL) clearance, and Criminal history information checks via fingerprinting with New York State Division of Criminal Justice Services
  • Ability to maintain clearances throughout employment
