Position Overview:

The Oncology Nurse Navigator plays a pivotal role in orchestrating patient care effectively and promptly, addressing and removing obstacles to treatment.

This position serves as a dedicated care coordinator throughout the patient's journey, assessing their physical, emotional, and social requirements to enhance outcomes, boost satisfaction, and minimize healthcare costs.

As an expert communicator, the Nurse Navigator provides timely and relevant guidance to patients seeking assistance with their healthcare concerns, whether over the phone or in person.

Key Responsibilities:

  • Conduct comprehensive patient assessments and reassessments to ensure appropriate transitions to the necessary level of care based on individual patient needs.
  • Rank health issues according to urgency, providing education and advice to clients while ensuring safe and effective care decisions in accordance with established protocols.
  • Exhibit compassion and empathy in all interactions with clients, whether face-to-face or via telephone.
  • Act as a patient advocate, facilitating access to care and promoting a patient-centered approach that respects individual choices and addresses healthcare needs.
  • Engage in direct patient care through visits, education, phone consultations, testing, and office procedures.
  • Collaborate with healthcare providers and staff to coordinate and manage patient care effectively.

Qualifications:

Education:
Registered Nurse with a Bachelor of Science in Nursing (BSN) preferred.

Licenses:
Must possess a current nursing license in New Mexico and hold valid BLS certification from the American Heart Association.

Skills:

  • Extensive knowledge of healthcare facilities, policies, and the full range of community patient care services.
  • Strong understanding of healthcare and social management principles.
  • Ability to coordinate various functions and activities to ensure maximum cooperation among service providers and patients.
  • Proficient in directing, instructing, and advising staff while effectively addressing daily challenges.
  • Excellent communication skills, both written and verbal, with a focus on interactive care coordination.
  • Understanding of cultural sensitivity and care measures appropriate for diverse populations.
  • Ability to work collaboratively with various levels of the organization, including clinical, financial, administrative, and medical staff.
  • Skilled in triaging patient calls and addressing issues within the scope of practice using established protocols.
  • Competent in documenting all patient interactions and care plans in electronic medical records.
  • Involved in the development and maintenance of individualized treatment summaries and care plans in accordance with applicable standards.

Experience:
A minimum of five years of experience in a healthcare environment, with a focus on quality care coordination and oncology preferred.

Work Environment:
Varied settings including inpatient, specialty, and primary care environments, with exposure to bloodborne pathogens.

Physical Requirements:
Ability to navigate various care settings, perform close work, and utilize a computer keyboard. Must possess strong customer service skills and the ability to manage stressful situations.
