This role serves as a dedicated care coordinator throughout the patient's journey, assessing their physical, emotional, and social requirements to enhance outcomes, improve satisfaction, and lower healthcare costs.

The Nurse Navigator excels in communication, whether over the phone or face-to-face, offering timely and relevant advice to patients seeking help with their health concerns.

Conducts thorough patient assessments and reassessments to ensure appropriate transitions to the necessary level of care tailored to individual needs.

This multifaceted position demands adaptability, independent judgment, and collaborative efforts across various disciplines.

Navigation entails prioritizing health issues based on urgency, educating patients, and making safe, effective decisions in accordance with established protocols, whether in person or via telephone.

Demonstrates a compassionate and empathetic approach to client interactions, both in person and over the phone.

Moreover, the Oncology Nurse Navigator acts as a patient advocate, ensuring access to care and fostering a patient-centered environment that empowers individuals, honors their choices, and addresses their healthcare needs.

The nurse engages in direct patient interactions through visits, education, phone consultations, testing, and office procedures. Collaborates with healthcare providers and staff to coordinate and manage patient care effectively.

Qualifications

Minimum Requirements:

Education:
Registered Nurse with a Bachelor of Science in Nursing (BSN) preferred.

Licenses:
Must possess a current nursing license in New Mexico and hold valid BLS certification from the American Heart Association.

Skills:

  • In-depth knowledge of healthcare facilities, policies, and the full range of community patient care services.
  • Strong understanding of healthcare and social management principles.
  • Ability to coordinate various functions and activities to ensure maximum cooperation and integration of services for patients.
  • Proficient in directing, instructing, and advising staff while effectively addressing daily challenges.
  • Excellent communication skills, both written and verbal, with a demonstrated interactive style of care coordination.
  • Understanding of sensitivity and cultural care measures suitable for diverse populations.
  • Ability to collaborate with various levels of the organization, including clinical, financial, administrative, and medical personnel.
  • Skilled in triaging calls from patients or families, addressing issues within the scope of practice using appropriate protocols.
  • Capable of making independent and collaborative decisions regarding specific triage plans.
  • Accurate documentation of all patient and provider interactions when medical direction is provided.
  • Development and documentation of patient care plans within Electronic Medical Records systems.
  • Participation in the creation, maintenance, and delivery of individual treatment summaries and care plans in line with applicable standards and guidelines.

  • Engagement in committee work and data collection for survivorship programs.
  • Continuous learning to stay updated on current practices in oncology, nursing, and healthcare.

Experience:

A minimum of five years of experience in a healthcare environment, with a focus on quality, care coordination, and oncology preferred.


Supervision:

  • Reports to: Clinical Nurse Manager, Medical Oncology

Work Environment:

  • Exposure to bloodborne pathogens in various settings, including inpatient, specialty, and primary care environments.

Physical Requirements:
Ability to navigate different care settings. Requires close visual attention, dexterity for writing and using a computer keyboard, and the capacity to manage stressful situations. Excellent customer service skills are essential.
