The Nurse Navigator excels in communication, whether over the phone or face-to-face, offering timely and relevant guidance to patients seeking help with their healthcare concerns.
Conducts thorough patient assessments and reassessments with the objective of guiding patients to the appropriate level of care tailored to their unique needs.
This role is complex, demanding adaptability, independent judgment, and collaborative efforts across multiple disciplines.Navigation entails prioritizing health issues based on urgency, educating patients, and making safe, effective decisions in accordance with established protocols, whether in person or via telephone.
Demonstrates a compassionate and empathetic approach to client interactions, both in person and remotely.Furthermore, the Oncology Nurse Navigator acts as a patient advocate, ensuring accessibility to care and fostering a patient-centered environment that empowers individuals, honors their choices, and addresses their healthcare needs.
The nurse engages in patient care through direct visits, educational initiatives, phone consultations, testing, and procedural tasks. Collaborates closely with healthcare providers and staff to manage and coordinate patient care effectively.Qualifications
Minimum Requirements:
Education:
Registered Nurse with a Bachelor of Science in Nursing (BSN) preferred.
Licenses:
Current Nursing License in New Mexico. Valid BLS certification from the American Heart Association is mandatory.
Skills:
- In-depth knowledge of healthcare facilities, policies, and issues, along with a comprehensive understanding of community patient care services.
- Strong grasp of healthcare and social management principles.
- Ability to coordinate various functions and activities to ensure maximum cooperation and integration among service providers and patients within a clinic setting.
- Capability to direct, instruct, and support staff while effectively addressing daily challenges.
- Proficient communication skills, both written and verbal, demonstrating an interactive approach to care coordination.
- Understanding and implementation of sensitivity and cultural care measures suitable for a diverse patient population.
- Ability to collaborate with various levels of the CHRISTUS organization, including clinical, financial, administrative, and medical personnel.
- Triages incoming calls from patients or families, addressing issues within the scope of practice using established protocols in a timely manner.
- Independently and collaboratively makes decisions regarding specific triage plans.
- Documents all patient and provider interactions when medical guidance is provided.
- Develops and records patient care plans in the Electronic Medical Records system(s).
- Assists in creating, maintaining, and delivering individualized treatment summaries and care plans for patients in accordance with applicable standards and guidelines within the Oncology Service Line.
- Participates in committee activities and data collection for survivorship programs.
- Maintains current knowledge of practices in oncology, nursing, and healthcare.
Experience:
A minimum of 5 years of experience in a healthcare environment, with related experience in quality care coordination and oncology preferred.
Supervision:
- Reports to: Clinical Nurse Manager, Medical Oncology
Work Environment:
- Exposure to bloodborne pathogens in various settings, including inpatient, specialty, and primary care environments.
Physical Requirements:
Ability to navigate different care settings. Requires close visual attention, dexterity for writing and using a computer keyboard, and the capacity to manage stressful situations. Must demonstrate exceptional customer service skills.
