They conduct detailed patient evaluations with the primary goal of facilitating a smooth transition to the appropriate level of care tailored to each patient's specific needs.
Core Responsibilities:
- Providing education and guidance on health-related matters
- Ensuring safe and suitable care transitions
- Collaborating effectively with other healthcare professionals
- Enhancing patient access to necessary services
- Engaging in direct patient care and educational initiatives
- Performing essential assessments and clinical procedures
- Overseeing and coordinating comprehensive patient care
Essential Qualifications:
Education: Registered Nurse with a Bachelor of Science in Nursing preferred
Licenses/Certifications: Valid New Mexico Nursing License and Basic Life Support (BLS) certification from the American Heart Association required
Key Skills:
- In-depth understanding of healthcare systems, policies, and community health services
- Expertise in healthcare and social management practices
- Exceptional communication and care coordination abilities
- Capacity to collaborate with diverse healthcare teams
- Minimum of 5 years of experience in a healthcare environment, emphasizing quality, care coordination, and oncology
Supervision Structure: Reports to the Clinical Nurse Manager, Medical Oncology
Work Environment: Exposure to bloodborne pathogens; various settings with dynamic and unpredictable circumstances
Physical Demands: Ability to navigate care environments, good vision, manual dexterity, and the capability to manage high-pressure situations while delivering outstanding customer service.
