We are seeking a highly skilled and compassionate Oncology Nurse Navigator to join our team at CHRISTUS Health. As a key member of our cancer care team, you will play a critical role in coordinating patient care, ensuring timely and efficient navigation through the healthcare system.
Key Responsibilities- Coordinate patient care, identifying and eliminating barriers to care, and assessing physical, psychological, and social needs.
- Function as a consistent care coordinator throughout the continuum of care, enhancing patient outcomes, increasing satisfaction, and reducing cost of care.
- Provide expert communication, providing timely, appropriate advice and/or guidance to patients and families.
- Perform patient assessment/reassessment, transitioning patients to the right level of care based on individual needs.
- Collaborate with healthcare providers and staff to coordinate and manage patient care.
- Participate in committee work, data collection, and analysis for survivorship programs.
- Maintain knowledge of current practices in oncology, nursing, and healthcare.
- Minimum of 5 years of experience in a healthcare setting, with related experience in quality, care coordination, and oncology preferred.
- Registered Nurse, BSN in Nursing, preferred.
- Current New Mexico Nursing License and BLS certifications issued by the American Heart Association required.
- Ability to move around settings of care.
- Requires close work, good vision, dexterity to write, and to use a computer keyboard.
- Ability to handle stressful situations.
