We are seeking a highly skilled and compassionate Oncology Nurse Navigator to join our team at CHRISTUS Health. Key Responsibilities- Patient Assessment and Care Coordination: Conduct thorough patient assessments to identify physical, psychological, and social needs, and develop personalized care plans to address these needs.
- Health Education and Advice: Provide expert advice and education to patients and their families on health issues related to cancer care, including treatment options, side effects, and symptom management.
- Collaboration with Healthcare Providers: Work closely with healthcare providers to ensure seamless care coordination and communication, and to facilitate timely and effective treatment.
- Patient Accessibility and Support: Facilitate patient accessibility to care, including arranging transportation, accommodations, and other support services as needed.
- Quality Improvement and Compliance: Participate in quality improvement initiatives and ensure compliance with regulatory requirements and organizational policies.
- Education and Certifications: Registered Nurse (RN) with a Bachelor of Science in Nursing (BSN) degree, and current New Mexico Nursing License and BLS certifications issued by the American Heart Association.
- Experience and Skills: Minimum of 5 years of experience in a healthcare setting, with a focus on quality, care coordination, and oncology. Strong communication and care coordination skills, with the ability to work collaboratively with various healthcare personnel.
**Bloodborne Pathogen Exposure**: This role may involve exposure to bloodborne pathogens, and the ability to work in a fast-paced, dynamic environment with variable and unpredictable situations.
**Physical Requirements**: Ability to move around care settings, good vision, dexterity, and the capability to handle stressful situations with excellent customer service skills.
