Position Summary

Admissions Nurse - Responsible for initiating patient care visits. Only Registered Nurse applicants will be considered.

Adoration Home Health is in search of an Admissions Nurse (RN) to become part of our dedicated team. Key Responsibilities
  • Deliver patient care in a safe and timely manner, adhering to best nursing practices as outlined by Adoration's quality assurance team.
  • Actively participate in weekly team meetings while adhering to current COVID safety protocols to ensure effective patient care coordination.
  • Implement nursing interventions that promote patient rehabilitation and achieve quality, outcome-driven results.
  • Provide compassionate nursing care while collaborating with other members of the interdisciplinary treatment team as necessary.
  • Adjust nursing care plans as required to help patients reach their maximum potential.
  • Administer medications as prescribed by the patient's Primary Care Physician, including instruction and monitoring.
  • Conduct patient assessments and collaborate with interdisciplinary team members to develop individualized care plans.
  • Supervise Licensed Practical Nurses and Home Health Aide staff as appropriate.
  • Communicate changes in patients' medical needs to the physician and nursing supervisor through proper documentation.
  • Ensure timely and accurate documentation of all patient care activities, including progress notes and discharge summaries.
  • Assist in creating and implementing personalized treatment plans that facilitate rapid recovery at home.
  • Adhere to safe nursing practices and report patient events to the nursing supervisor promptly.
  • Contribute to the maintenance and updating of nursing, pharmacy, infection control, and health and safety policies as requested.
  • Demonstrate strong self-motivation and comfort in interacting with a supportive clinical team.
  • Utilize electronic medical record and documentation software (WellSky) effectively.
  • Perform other related tasks as assigned.
Qualifications
  • Registered Nursing Degree (Associate or Bachelor) from an accredited nursing college with current unrestricted registration and licensure in the applicable state.
  • Minimum of one year of nursing experience in a patient care environment; experience in home health or geriatrics is preferred.
  • Valid driver's license with an acceptable driving record and proof of car insurance in accordance with company policy.
  • Current CPR certification.
