PRIMARY RESPONSIBILITIES:
Evaluate medical records for all patient admissions, perform medication reconciliation, and secure admission orders from attending physicians. Coordinate emergency transfers for patients presenting with critical medical conditions beyond the facility's capabilities. Assess referrals from hospitals, care facilities, and community organizations for medical and behavioral severity and respond in accordance with facility protocols. Address inquiries regarding the facility and provide suitable recommendations. Conduct evaluations to ascertain the appropriate level of care required within the facility. Collaborate with medical and psychiatric staff to ensure accurate recommendations and admissions. Oversee the admissions process for incoming patients, including evaluations, consent acquisition, and inventory of personal belongings. Perform insurance benefit verifications and obtain initial pre-authorization for treatment and admission. Monitor patient conditions throughout the admissions process in alignment with facility policies and/or physician directives. Utilize various web-based platforms to track and review referrals. Communicate anticipated admissions to designated internal personnel promptly. Ensure all clinical information from referral sources or patients (including medical comorbidity details) is collected, when feasible, prior to patient admission. Schedule and conduct pre-admission assessments when applicable, consult with the admitting physician, and communicate disposition recommendations to the patient or their family. Exhibit proficiency in preparing and maintaining appropriate medical record documentation that will facilitate authorization at the requested level of care from the payor. Complete initial pre-authorization for treatment and admission prior to admission, when feasible, and within payor timeframe guidelines. Admit the patient into the registration and accounting system. Complete all admission and consent forms with the patient. Maintain a positive, empathetic, and professional demeanor towards customers at all times. When patient needs are unmet, acknowledge and work to resolve complaints. Recognize that patient safety is paramount. When a patient's needs are not fulfilled, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority. Coordinate care for patients who are not being admitted and ensure they receive appropriate follow-up care and referrals. Demonstrate urgency regarding the significance of patient safety and provide exceptional customer service. Responsible for conducting safety checks and ensuring that supervision is carried out at 15-minute intervals, as indicated in special precautions, or in accordance with individualized supervision guidelines as necessary.ADDITIONAL RESPONSIBILITIES:

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
Minimum of an Associate's degree in Nursing. At least one year of experience working with individuals in a clinical or observational capacity, preferably within the specialty area of the hiring facility.LICENSES/DESIGNATIONS/CERTIFICATIONS:
Clear and active nursing license in the state. CPR and de-escalation/restraint certification required (training available upon hire and offered by facility). First aid certification may be required based on state or facility regulations.
