Acute Care Admissions Nurse

KEY RESPONSIBILITIES:
- Assess medical documentation for all patient admissions, ensuring medication reconciliation is completed and admission orders are secured from healthcare providers.
- Coordinate emergency transfers for patients with critical medical conditions that exceed the facility's capabilities.
- Evaluate referrals from hospitals, healthcare facilities, and community organizations for both medical and behavioral health needs, responding in accordance with facility policies.
- Address inquiries regarding the facility and provide suitable recommendations based on patient needs.
- Conduct thorough assessments to determine the appropriate level of care required for incoming patients.
- Collaborate with medical and psychiatric staff to ensure proper recommendations and facilitate admissions.
- Oversee the admissions process for new patients, including assessments, consent procedures, and inventory of personal belongings.
- Perform insurance benefit verifications and obtain initial pre-authorization for treatment and admissions.
- Monitor patient status throughout the admissions process, adhering to facility policies and physician directives.
- Utilize various web-based platforms to track and manage referrals efficiently.
- Communicate anticipated admissions to designated internal staff promptly.
- Ensure that all clinical information from referral sources or patients is collected prior to admission whenever feasible.
- Schedule and conduct pre-admission assessments, consult with the admitting physician, and relay disposition recommendations to patients or their families.
- Maintain accurate medical record documentation to support authorization at the requested level of care.
- Complete initial pre-authorization for treatment and admission prior to patient arrival, when possible, and within payer guidelines.
- Register patients in the system, completing all necessary admission and consent forms with them.
- Exhibit a positive, empathetic, and professional demeanor towards all customers. Address and resolve complaints when patient needs are unmet, prioritizing patient safety.
- Ensure appropriate follow-up care and referrals for patients who are not admitted.
- Demonstrate urgency regarding patient safety and provide exceptional customer service.
- Conduct safety checks and ensure supervision is maintained at required intervals as per individualized guidelines.
ADDITIONAL DUTIES:
Perform other related tasks as assigned.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- A minimum of an Associate's degree in Nursing is required.
- At least one year of experience in a clinical or observational role, preferably within the relevant specialty area.
LICENSURE AND CERTIFICATION:
- Must possess a clear and active nursing license in the state.
- CPR and de-escalation/restraint certification are required (training provided upon hire).
- First aid certification may be necessary based on state or facility requirements.
