Acute Care Admissions Nurse

KEY RESPONSIBILITIES:
- Evaluate medical records for all patient admissions, perform medication reconciliation, and secure admission orders from healthcare providers.
- Coordinate emergency transfers for patients with urgent medical needs that exceed the facility's capabilities.
- Assess referrals from hospitals, healthcare facilities, and community organizations for medical and behavioral needs, responding within established facility guidelines.
- Address inquiries regarding the facility and provide suitable recommendations.
- Conduct evaluations to ascertain the appropriate level of care required within the facility.
- Collaborate with medical and psychiatric staff to ensure accurate recommendations and admissions.
- Facilitate the admissions process for new patients, including assessments, consent acquisition, and inventory of personal belongings.
- Perform insurance benefit verifications and obtain initial pre-authorization for treatment and admission.
- Monitor patient status throughout the admissions process in accordance with facility policies and physician directives.
- Utilize various web-based platforms to track and manage referrals.
- Promptly communicate anticipated admissions to designated internal personnel.
- Ensure all clinical information from referral sources or patients is collected, when feasible, prior to patient admission.
- Schedule and conduct pre-admission assessments as necessary, consult with the admitting physician, and relay disposition recommendations to patients or their families.
- Exhibit proficiency in preparing and maintaining accurate medical record documentation to facilitate authorization at the requested level of care.
- Complete initial pre-authorization for treatment and admission prior to patient arrival, when feasible, adhering to payer timeframe guidelines.
- Register the patient in the system and complete all necessary admission and consent forms with the patient.
- Maintain a positive, empathetic, and professional demeanor towards all customers. Address and resolve complaints when patient needs are unmet, recognizing that patient safety is paramount.
- Ensure that patient safety remains a top priority, addressing any unmet needs promptly.
- Coordinate care for patients not being admitted, ensuring they receive appropriate follow-up care and referrals.
- Demonstrate urgency regarding patient safety and deliver exceptional customer service.
- Conduct safety checks and ensure supervision is maintained at regular intervals as required.
ADDITIONAL RESPONSIBILITIES:
Carry out other functions and tasks as assigned.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Associate's degree in Nursing is required.
- A minimum of one year of experience in a clinical or observational role, preferably in the relevant specialty area.
CERTIFICATIONS/LICENSES:
- Active nursing license in the state is required.
- CPR and de-escalation/restraint certification are mandatory (training provided upon hire).
- First aid certification may be required based on state or facility regulations.
