Key Responsibilities

The Registered Nurse in Home and Community-Based Services is responsible for delivering and overseeing nursing and personal care to patients in their residences, as directed by their physician or authorized by state regulations, while adhering to relevant laws and agency policies. The RN collaborates with other healthcare professionals to ensure continuity of care by effectively communicating with physicians, agency staff, patients, families, case managers, and community resources.

Primary Duties Include:

  • Delivering high-quality clinical services in accordance with the care plan and infection control standards, coordinating with the patient’s care team from admission to discharge.
  • Conducting clinical nursing assessments as mandated by federal and/or state regulations and payer requirements.
  • Verifying patient eligibility and medical necessity for services based on payer guidelines and agency policies.
  • Creating and updating individualized care plans in collaboration with other community providers.
  • Ensuring that the frequency and duration of care plans align with patient needs, making adjustments as necessary with physician consent.
Qualifications

Experience Required:

  • A minimum of one year of experience as a Registered Nurse is preferred.
Licensure Requirements:

  • Must possess a current RN license in the state of practice.
  • Valid driver's license, vehicle insurance, and access to reliable transportation.
  • Current CPR certification is mandatory.
