KEY RESPONSIBILITIES:

  • Evaluate medical records for all patient admissions, ensuring medication reconciliation is completed and admission orders are secured from physicians.
  • Coordinate emergency transfers for patients with urgent medical needs that exceed the facility's capabilities.
  • Assess referrals from hospitals, facilities, and community organizations for both medical and behavioral health needs, responding within established policy timelines.
  • Address inquiries regarding the facility and provide suitable recommendations.
  • Conduct thorough assessments to determine the appropriate level of care required for incoming patients.
  • Collaborate with medical and psychiatric staff to ensure accurate recommendations and admissions.
  • Oversee the admissions process for new patients, including assessments, consent procedures, and inventory of personal belongings.
  • Perform insurance benefit verifications and obtain initial pre-authorization for treatment and admission.
  • Monitor patient progress throughout the admissions process in accordance with facility policies and physician directives.
  • Utilize various web-based platforms to track and manage referrals effectively.
  • Promptly communicate anticipated admissions to designated internal staff.
  • Ensure all clinical information from referral sources or patients is collected prior to admission, when feasible.
  • Schedule and conduct pre-admission assessments, consult with the admitting physician, and communicate recommendations to patients or their families.
  • Maintain accurate medical record documentation to facilitate authorization at the requested level of care.
  • Complete initial pre-authorization for treatment and admission prior to patient arrival, when possible, adhering to payer guidelines.
  • Register patients in the system and complete all necessary admission and consent forms.
  • Exhibit a positive, empathetic, and professional demeanor towards all customers. Address and resolve complaints when patient needs are unmet, prioritizing patient safety.
  • Ensure that patient safety remains a top priority, acknowledging and addressing any unmet needs.
  • Coordinate care for patients not admitted, ensuring they receive appropriate follow-up and referrals.
  • Demonstrate urgency regarding patient safety and deliver exceptional customer service.
  • Conduct safety checks and ensure supervision is maintained at required intervals as per individual guidelines.

ADDITIONAL RESPONSIBILITIES:

Carry out other duties and tasks as assigned.

EDUCATION/EXPERIENCE/SKILLS REQUIRED:

  • Associate's degree in Nursing or higher.
  • At least one year of experience in a clinical or observational role, preferably within the relevant specialty.

CERTIFICATIONS/LICENSES:

  • Active nursing license in the state.
  • CPR and de-escalation/restraint certification required (training provided upon hire).
  • First aid certification may be necessary based on state or facility requirements.
