Job Title: Diabetes Care Specialist

Parkland Health and Hospital System is seeking a highly skilled Diabetes Care Specialist to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and education to individuals living with diabetes.

Job Summary:

The Diabetes Care Specialist will be responsible for providing comprehensive diabetes self-management education and support to patients and their families. This includes assessing patient needs, developing individualized care plans, and providing education on diabetes management, nutrition, and lifestyle changes.

Responsibilities:
  • Provide individualized diabetes self-management education and support to patients and their families
  • Assess patient needs and develop individualized care plans
  • Provide education on diabetes management, nutrition, and lifestyle changes
  • Collaborate with the multidisciplinary diabetes care team to optimize patient care
  • Participate in the development of health professional, patient-centered, and community education programs and resources
Requirements:
  • Graduate from an accredited school of nursing or bachelor's degree in Food and Nutrition
  • Three years of clinical experience in the direct care of people with diabetes and associated conditions
  • Current, valid RN license or temporary RN license from the Texas Board of Nursing
  • Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) from the Certification Board for Diabetes Care and Education (CBDCE)
Preferred Qualifications:
  • Bilingual (oral and writing) in Spanish
  • Basic computer skills with a working knowledge of data base programs, data report analysis software, and electronic medical record charting
