Job Summary

The Diabetes Care Specialist will provide comprehensive diabetes self-management education and support to patients, families, and caregivers. This role will involve assessing patient needs, developing individualized care plans, and delivering evidence-based education and interventions to promote optimal diabetes management.

Responsibilities
  1. Assess patient needs and develop individualized care plans to promote optimal diabetes management.
  2. Deliver evidence-based diabetes self-management education and support to patients, families, and caregivers.
  3. Collaborate with multidisciplinary teams to optimize patient care and outcomes.
  4. Participate in quality improvement initiatives to enhance diabetes care and education.
  5. Stay current with best practices in diabetes care and education through ongoing professional development.
Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing or a related field.
  • Current RN licensure or RD certification.
  • At least three years of experience in diabetes care and education.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
