Job SummaryThe Diabetes Care Specialist will provide comprehensive diabetes self-management education and support to patients, families, and caregivers. This role will involve assessing patient needs, developing individualized care plans, and delivering evidence-based education and interventions to promote optimal diabetes management.
Responsibilities- Assess patient needs and develop individualized care plans to promote optimal diabetes management.
- Deliver evidence-based diabetes self-management education and support to patients, families, and caregivers.
- Collaborate with multidisciplinary teams to optimize patient care and outcomes.
- Participate in quality improvement initiatives to enhance diabetes care and education.
- Stay current with best practices in diabetes care and education through ongoing professional development.
To be successful in this role, you will need:
- A graduate degree in nursing or a related field.
- Current RN licensure or RD certification.
- At least three years of experience in diabetes care and education.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
