Advanced Practice Provider

Parkland Health and Hospital System (PHHS) is seeking a highly skilled Advanced Practice Provider to join our team in the Inpatient/Outpatient Diabetes department. As a key member of our healthcare team, you will provide high-quality patient care, education, and support to patients with diabetes.
Responsibilities- Provide comprehensive patient care, including history taking, physical exams, diagnoses, treatment, and referrals.
- Develop and implement individualized plans of care, taking into account patient needs, preferences, and goals.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and optimal patient outcomes.
- Stay up-to-date with the latest developments, advancements, and trends in diabetes care, and apply this knowledge to improve patient outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellence in patient care.
- Master's degree in nursing or physician assistant studies from an accredited institution.
- Current licensure as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in the state of Texas.
- Active certification in a relevant specialty, such as Family Nurse Practitioner (FNP) or Physician Assistant (PA-C).
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong problem-solving and critical thinking skills, with the ability to analyze complex patient data and develop effective plans of care.
