Diabetes Care Specialist

Parkland Health and Hospital System is seeking a highly skilled Diabetes Care Specialist to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and education to individuals living with diabetes.
Job Summary:
The Diabetes Care Specialist will be responsible for providing comprehensive diabetes self-management education and support to patients, as well as collaborating with the multidisciplinary diabetes care team to optimize patient outcomes. This role requires a strong understanding of diabetes management, excellent communication skills, and the ability to work effectively with patients, families, and healthcare providers.
Responsibilities:
- Provide individualized diabetes self-management education and support to patients, including assessment, goal setting, and education on diabetes management, nutrition, and physical activity.
- Collaborate with the multidisciplinary diabetes care team to develop and implement individualized care plans, including medication management, glucose monitoring, and lifestyle modifications.
- Conduct comprehensive patient assessments, including medical history, physical examination, and laboratory results, to identify areas for improvement in diabetes management.
- Develop and implement educational programs for patients, families, and healthcare providers on diabetes management, nutrition, and physical activity.
- Participate in quality improvement initiatives to enhance patient outcomes and improve the overall quality of care.
Requirements:
- Graduate from an accredited school of nursing or a related field.
- Current RN license or temporary RN license from the Texas Board of Nursing.
- Certified Diabetes Care and Education Specialist (CDCES) or Certified Diabetes Educator (CDE) certification.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
Preferred Qualifications:
- Bilingual (oral and written) in Spanish.
- Experience working with diverse patient populations.
- Knowledge of diabetes management, nutrition, and physical activity.
