About the Role

We are seeking a highly skilled and compassionate Resident Care Coordinator to join our team at Christian Horizons. As a key member of our nursing service team, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Assess Resident Care Needs: Conduct thorough assessments of resident care needs and develop individualized care plans to meet their unique requirements.
  • Coordinate Care Conferences: Schedule and conduct care conferences to ensure the coordination of all departments and ensure seamless care delivery.
  • Conduct MDS Assessments: Interview residents and their families to gather information for the Minimum Data Set (MDS) assessment, ensuring timely and accurate submissions.
  • Review Medical Records: Review medical records and other paperwork to collect information related to resident health status and MDS records.
  • Evaluate Resident Care: Evaluate resident care in relation to individualized resident needs, family involvement, and physician plans of care.
  • Direct and Organize Assessments: Direct, organize, and complete required resident assessments and related documentation for Medicare, Medicaid, and private insurance reimbursement.
  • Ensure Quality Customer Service: Provide exceptional customer service to residents, families, staff, and external customers, ensuring a positive experience.
  • Monitor Compliance: Monitor nursing service compliance with federal, state, local, and corporate regulations.
  • Participate in Continuing Education: Attend and participate in continuing education programs to stay up-to-date on changes in the profession.
Requirements
  • Graduate of an Accredited School of Nursing: Must be a graduate of an accredited school of nursing.
  • Current State License: Must hold a current state license as an LPN or RN.
  • Clear Understanding of MDS: Must have a clear understanding of the Minimum Data Set (MDS) and its requirements.
  • Experience with MDS: Preferably 1 year of experience with MDS assessments.
  • Leadership Qualities: Must demonstrate leadership qualities, good communication skills, and a desire to continuously learn.
  • Background Check: Must pass a background check before and during employment, as defined by Christian Horizons' background check policy.
