About the Role

We are seeking a highly skilled and compassionate Resident Care Coordinator to join our team at Christian Horizons. As a key member of our nursing service team, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Assess Resident Care Needs: Conduct thorough assessments of resident care needs and develop individualized care plans to meet their unique requirements.
  • Coordinate Resident Care Conferences: Schedule and conduct resident care conferences to ensure the coordination of all departments and ensure seamless care delivery.
  • Conduct MDS Assessments: Conduct Minimum Data Set (MDS) assessments on all residents, ensuring timely and accurate submissions.
  • Review Medical Records: Review medical records and other paperwork related to resident health status to collect information for MDS records.
  • Evaluate Resident Care: Evaluate resident care as related to individualized resident needs, family involvement, and physician plans of care.
  • Direct and Organize Resident Assessments: Direct, organize, and complete required resident assessments and related documentation for Medicare, Medicaid, and private insurance reimbursement.
  • Ensure Quality Customer Service: Provide exceptional customer service to residents, families, staff, and external customers.
  • Monitor Compliance: Monitor responsibility for nursing service compliance with federal, state, local, and corporate regulations.
Requirements
  • Graduate of an Accredited School of Nursing: Must be a graduate of an accredited school of nursing.
  • Current State License: Must hold a current state license as an LPN or RN.
  • Clear Understanding of MDS: Must have a clear understanding of MDS and at least 1 year of experience with MDS preferred.
  • Leadership Qualities: Must demonstrate dependability, cooperation, and interest in the care of the elderly, as well as display leadership qualities, good communication skills, and a desire to continuously learn.
