Adult Mental Health Community Based Case Manager

Zepf Center is a leading provider of behavioral health and substance use disorder services in Northwest Ohio, serving the community for nearly 50 years. We are seeking a Full-Time Community-Based Case Manager to join our team and assist Adult Mental Health clients in achieving their goals.
Key Responsibilities- Evaluate client strengths and needs, collaborating with the client and relevant team members to develop a comprehensive care coordination and treatment strategy.
- Track and assess client progress towards achieving goals, reassessing and adjusting plans every 90 days as needed.
- Maintain regular contact with clients, providing support and advocacy to ensure stability and access to community resources.
- Link and refer clients to other services, programs, and agencies as needed, ensuring timely access to care.
- Collaborate with hospital and agency medical staff to facilitate discharge planning and coordination of services.
- Assist clients in tracking and keeping appointments, completing required paperwork in a timely manner.
- High School Diploma or GED required, with demonstrated mental health experience; or Bachelor's degree in social work or three years of mental health experience in behavioral health - preferred.
- CDCA certification required for SUD.
- Valid driver's license and acceptable driving record.
