About the Role

Community of Hope is seeking a dedicated and compassionate Housing Stability Case Manager to join our team. As a key member of our staff, you will play a vital role in supporting families and individuals experiencing homelessness in Washington, DC. Your primary responsibility will be to provide housing stability and income-focused case management services to families assigned to your caseload.

Key Responsibilities
  • Develop and implement case management plans that address the unique needs of each family, including finding stable housing, increasing income, and family stabilization and support.
  • Meet weekly and as needed with families on your caseload to review progress, provide guidance, and make referrals to community resources as needed.
  • Collaborate with families and Housing Specialists to assist with housing search and placement, including accompanying families to view units and advocating for families with landlords.
  • Network with community resources to provide referrals and support for families, including employment, substance abuse treatment, mental health treatment, and training.
  • Complete required assessments with families and maintain accurate records of their progress.
  • Communicate regularly with Youth Specialists to ensure children's needs are addressed in the parent's case management plan.
  • Obtain signed release of information forms and communicate with staff at treatment programs, substance abuse programs, and vocational programs.
Requirements
  • Bachelor's degree in social work or related field required or 4 years social service experience.
  • For Bachelor's of Social Work degree or Master's of Social Work degree holders, the appropriate DC license must be obtained prior to start date.
  • Minimum one year work experience in social service or related field preferred.
  • Prior experience working with individuals or families experiencing homelessness and/or living in poverty strongly preferred.
  • Strong interpersonal, conflict-resolution, and organizational skills required.
  • Ability to work independently and as a team required.
  • A valid driver's license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.
