Behavioral Health Care Coordinator

CareOregon is seeking a highly skilled Behavioral Health Care Coordinator to join our team. As a key member of our Care Management department, you will play a critical role in providing assessment, care planning, and service coordination for members with complex or special health care needs.
Key Responsibilities- Conduct comprehensive assessments of members' behavioral health needs, including mental status examinations and formulation of multiaxial DSM impressions.
- Develop and implement individualized plans of care in collaboration with members, providers, and other relevant parties.
- Coordinate with primary care and community health care providers, as well as Aging and People with Disabilities caseworkers, to ensure timely access to services.
- Provide direction and guidance to non-clinical Care Coordination staff involved with members.
- Collaborate with community partners to develop plans to address complex care needs and monitor and evaluate plans of care for optimal outcomes.
- Master's degree in social work, counseling, or other behavioral health field.
- Minimum 2 years' experience in mental health and/or drug and alcohol treatment for the population being served.
- Knowledge of current version of the Diagnostic and Statistical Manual of Mental Disorders (DSM) criteria for mental health and substance dependence/abuse diagnoses.
- Ability to conduct psychosocial assessments, including history of mental and physical disabilities, alcohol and drug use, past mental health and criminal justice contacts, family and social relationships, and level of functioning.
- Current Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT).
- Experience with a similar population in health plan case management/care coordination or behavioral health integration in a person-centered primary care home.
- Experience administering work relating to the Oregon Health Plan (OHP) (Medicaid) and the Centers for Medicare and Medicaid Services (CMS) (Medicare) benefits.
