Position Summary

Role Responsibilities:

Health Navigation involves advocating for and guiding patients through the complexities of the medical and behavioral health systems. Our navigators provide personalized assistance with appointments, medication management, and health literacy. The typical case load is one navigator for every 15-25 patients. We offer comprehensive support to our patients, from locating healthcare providers to understanding insurance options and managing chronic health issues. Our ultimate aim is to ensure that patients receive optimal care.

Key Duties:

  • Facilitate prescription fulfillment for patients
  • Coordinate and schedule patient appointments
  • Accompany patients to all scheduled appointments
  • Monitor medication refills
  • Document patient interactions and appointments in an Electronic Medical Record system
  • Communicate information between healthcare providers
  • Avoid redundant services and testing
  • Promote the use of preventive services and reduce emergency department visits
  • Work collaboratively on patient-specific goals
  • Connect patients with Social Determinants of Health (SDOH) providers
  • Enhance the safety of patients' living environments
  • Provide transportation to essential appointments using a personal vehicle (generous auto-allowance for fuel and maintenance provided)
  • Complete additional tasks as assigned by the Direct Supervisor
  • Offer Peer Support services

Assessment of Skills:

  • Weekly individual supervision sessions
  • Continuous performance monitoring
  • Quarterly reviews and annual evaluations

Prerequisites for Employment:

  • AZ Fingerprint Card
  • CPR/First Aid certification
  • TB Test/Flu Shot

Probationary Period:

The initial 90 days of employment will serve as a probationary period, during which employment may be terminated without notice during the first month, and with one week's notice thereafter. Employees who transition to a new role within the organization may be offered the opportunity to return to their previous position if available.

During this period, new hires will receive:

  • Comprehensive training and support from their supervisor
  • Weekly feedback sessions with the supervisor
  • Assistance in meeting performance standards
  • A written performance evaluation at the end of the probationary period

Upon successful completion of the probationary period, employees will achieve permanent status. Employment remains at-will and can be terminated at any time with or without cause, subject to company policies.

Qualifications:

  • Minimum age of 21
  • 39 months of clear Motor Vehicle Record
  • Auto insurance coverage
  • Ability to pass a criminal background check
  • Associate or bachelor's degree, or equivalent work experience
  • Experience with diverse, chronically homeless, and vulnerable populations in healthcare settings (preferred)
  • Access to a reliable four-door vehicle
  • Current Arizona driver's license
  • Bilingual in Spanish is highly desirable but not required
