Job Overview:
The Care Navigator is essential in delivering a positive and inviting experience for our members and their families.
This role involves guiding members from their arrival to their departure, ensuring that each member has a pleasant experience and that their healthcare needs are addressed.
The ideal candidate will possess a strong customer service focus, a passion for senior care, and a commitment to going above and beyond to ensure a positive experience for our members.
Key Responsibilities:
- Greet members upon arrival and create a safe, welcoming environment, guiding them through a smooth check-in process.
- Update member information in the electronic medical records system.
- Schedule appointments and coordinate referrals for a multidisciplinary care team.
- Assist members with completing paperwork through electronic kiosks as necessary.
- Request medical records and upload documentation to the electronic medical records system.
- Address inquiries from prospective and existing members, as well as their family members.
- Manage the phone line by responding to incoming calls, taking detailed messages, and relaying important information to the appropriate individuals.
- Support center events as needed.
Required Skills and Qualifications:
- Exceptional customer service skills, characterized by a positive and welcoming attitude.
- A strong commitment to providing a quality experience for our senior members.
- Ability to effectively engage with diverse populations, including staff, providers, members, family members, insurance carriers, vendors, and the public.
- Familiarity with medical terminology, insurance processes, and/or electronic medical record systems.
- High level of organization and attention to detail.
- Strong written and verbal communication skills.
- Proficient in PC skills, including Microsoft Office Suite.
- Ability to maintain professionalism and adaptability in a dynamic work environment.
- Fluency in Spanish or other languages spoken by our community members (where applicable).
Education and Experience:
- An associate's degree is preferred, or equivalent experience.
- A minimum of one year of experience in a clinical setting or a similar role.
- Experience with EMR systems is advantageous.
