ROLE SUMMARY

Responsibilities:

The Patient Care Coordinator plays a crucial role in advocating for and assisting patients as they navigate the healthcare and behavioral health systems. This position offers personalized support with appointments, medication management, and health education. The typical case load consists of 15-25 patients per coordinator. Our goal is to provide comprehensive assistance, from locating healthcare providers to understanding insurance options and managing chronic health issues. By offering tailored support, we ensure our patients receive timely and quality healthcare.

Key Duties Include:

  • Facilitating prescription fulfillment for patients
  • Coordinating all patient appointments
  • Accompanying patients to their appointments
  • Monitoring medication refills
  • Documenting patient interactions and appointments in an Electronic Medical Record system
  • Communicating effectively between healthcare providers
  • Avoiding unnecessary duplication of services and tests
  • Encouraging the use of preventive healthcare services and reducing emergency department visits
  • Working collaboratively on patient-specific goals
  • Connecting patients with social determinants of health providers
  • Enhancing the safety of patients' living environments
  • Providing transportation to essential appointments using a personal vehicle (auto allowance for fuel and maintenance provided)
  • Completing additional tasks as assigned by the supervisor
  • Delivering Peer Support services

Evaluation of Navigation Skills:

  • Weekly individual supervision sessions
  • Continuous performance monitoring
  • Quarterly reviews and annual evaluations

Pre-employment Requirements:

  • Arizona Fingerprint Card
  • CPR/First Aid certification

PROBATIONARY PERIOD

The initial 90 days of employment will serve as a probationary period, during which employment may be terminated without notice during the first month, and with one week's notice thereafter. Employees who transition to a new role within the company and do not succeed may be considered for their previous position when available.

Gellert Health reserves the right to extend this period for an additional 90 days. Performance and suitability for continued employment will be assessed during this time.

New hires will receive:

  • Comprehensive training and guidance from their supervisor
  • Weekly feedback sessions with the supervisor
  • Support for performance improvement
  • A written performance evaluation at the end of the probationary period

Upon successful completion of the probationary period, employees will attain permanent status. However, employment remains at-will and can be terminated at any time.

QUALIFICATIONS:

  • Minimum age of 21
  • 39 months of a clear Motor Vehicle Record
  • Valid auto insurance
  • Ability to pass a criminal background check
  • Associate or bachelor's degree, or equivalent work experience
  • Experience with diverse, chronically homeless, and vulnerable populations in healthcare settings (preferred)
  • Reliable four-door vehicle
  • Current Arizona driver's license
  • Bilingual in Spanish is a plus but not required
