Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, develop care plans, and provide education to patients and their families. You will work collaboratively with our interdisciplinary team to ensure seamless care coordination and optimal patient outcomes.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
  • Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
  • Master's degree required or commensurate experience and satisfactory completion of NP licensure.
  • Master's degree required or commensurate experience and satisfactory completion of PA licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certified by the NCCPA, AANP, ANCC, or the AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant.
  • Ability to travel, valid state driver's license, and adequate automobile insurance coverage.
