As a Hospice Care Coordination Specialist, you will play a pivotal role in the daily operations of our office, ensuring seamless collaboration among the Inter-disciplinary Team while overseeing the clinical initiatives for our patients. Our Hospice Care Coordination Specialists serve as the central point of organization for both the office and clinical teams, guiding families through the end-of-life journey and beyond. For many, this role transcends a mere job; it is a profound calling.

Position Overview

The Hospice Care Coordination Specialist is tasked with exemplifying the core values of Compassion, Integrity, and Excellence, while actively promoting the FC Compassus philosophy, grounded in our 6 Pillars of Success. This role involves adhering to the Code of Ethical Conduct and fostering positive relationships within the organization, across all departments, and with external partners. The Specialist is responsible for managing patient charts, which includes initiating the chart at admission and maintaining documentation until the patient is discharged and the chart is officially closed, in compliance with Medicare/Medicaid regulations. Additionally, the Specialist will ensure timely communication of patient information to all members of the Inter-disciplinary Team (IDT).

Key Responsibilities
  • Accurately enters the following information:
    • Referral intake data.
    • Details of newly admitted patients.
    • Colleague time and visit counts, including mileage on daily labor tools.
    • Patient visit documentation linked to Time and Travel Logs.
    • Updated patient demographic details.
    • All patient medications (under the supervision of a registered nurse).
  • Inputs patient data into Health Trans and completes the pharmacy instruction sheet; faxes the instruction sheet along with the medication list to the pharmacy.
  • Establishes and maintains patient charts.
  • Ensures all admission documentation is properly signed and dated, including attending physician/medical director CTI and orders to admit.
  • Monitors DME and medical supply orders, reporting trends to the management team.
  • Logs and tracks physician orders for timely submission and return.
  • Verifies the accuracy and completeness of all time logs, ensuring they correspond with clinical visit notes and relevant attachments.
  • Files documents appropriately within patient charts and organizes charts as necessary.
  • Documents changes in patient care levels accurately.
  • Collects and organizes recertification/decertification paperwork, notifying relevant providers of patient discharges.
  • Requests medical records and hospital inpatient charts, processing provider notifications as needed.
  • Manages the transition of deceased/discharged patient charts from active status to established files.
  • Facilitates communication among IDT members.
  • Engages effectively with IDT members, patients, families, and the community.
  • Handles incoming calls, taking accurate messages and transferring calls as necessary.
  • Assists the Director of Clinical Services with pre- and post-IDT activities, including agendas and updated plans of care (UPOCs).
  • Maintains updated plans of care in patient charts, returning originals to nurses for signatures and re-filing them appropriately.
  • Updates IDT agendas and patient rosters.
  • Mails or faxes UPOCs to attending physicians.
  • Performs additional duties as assigned.
Qualifications
  • High school diploma or GED is required.
  • Two (2) to three (3) years of relevant experience and/or training is highly preferred.
Skills Required
  • Mathematical Skills: Proficient in basic arithmetic operations and the ability to compute rates, ratios, and percentages.
  • Language Skills: Capable of reading, analyzing, and interpreting business publications, professional journals, and technical procedures. Strong written and verbal communication skills are essential.
  • Other Skills and Abilities: Proficient in understanding, reading, writing, and speaking English. Embraces the philosophy of hospice care and communicates effectively with healthcare professionals. Maintains high integrity, particularly regarding confidential patient information.
Physical Requirements and Work Environment: The physical demands outlined here are representative of those that must be met by a colleague to successfully perform the essential functions of this role. While performing the duties of this position, the colleague is regularly required to use hands for various tasks, communicate verbally, and hear effectively. The colleague frequently needs to stand, sit, and reach with hands and arms. Occasional walking, climbing, balancing, stooping, kneeling, crouching, or crawling may be required, along with the ability to lift and/or move more than 25 pounds. Specific vision abilities required include close vision. The work environment characteristics described here are typical for this role, with potential exposure to infectious diseases through interactions with clinical caregivers. The noise level in the work environment is generally quiet.
