Role Responsibilities

The Hospice Nurse Practitioner plays a vital role in delivering palliative care, symptom management, and supportive services tailored to the needs of hospice patients, as directed by the referring physician.

  • Conducts in-person visits to eligible hospice patients within designated timeframes and communicates clinical findings to the Hospice Medical Director.
  • Collaborates with the Hospice Medical Director as necessary, keeps the primary physician informed of recommended services, and works alongside other healthcare providers as needed.
  • Develops and coordinates individualized care plans, documenting issues, goals, interventions, and patient/family responses to hospice services.
  • Educates and guides patients and families in self-care techniques when appropriate.
  • Keeps precise and relevant clinical records regarding patient conditions, reporting any changes or concerns to the Administrator and Hospice Medical Director.

Experience and Qualifications

Experience Requirements:

  • A minimum of five years of nursing experience, with a preference for clinical expertise in oncology, hospice, geriatrics, or intensive care.

Licensure Requirements:

  • Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
  • Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
  • A valid driver's license and vehicle insurance, along with access to reliable transportation.
  • Current CPR certification is mandatory.

Additional State-Specific Requirements:

  • No additional state-specific requirements.

Preferred Licensure:

  • Palliative Care ARNP is preferred.

Skill Requirements:

  • Familiarity with the interdisciplinary team approach in healthcare.
