Role Responsibilities
The Hospice Nurse Practitioner plays a crucial role in delivering palliative care, managing symptoms, and providing supportive services tailored to the needs of hospice patients as directed by their referring physician.
- Conducts in-person visits to eligible hospice patients within designated timeframes and communicates clinical findings to the Hospice Medical Director.
- Collaborates with the Hospice Medical Director as necessary, updates the primary physician on recommended services, and works alongside other healthcare providers as needed.
- Develops and coordinates individualized care plans, documenting issues, objectives, interventions, and responses from patients and families regarding hospice care.
- Educates and guides patients and families in self-care practices when appropriate.
- Keeps precise and relevant clinical documentation concerning the patient's status and communicates any concerns to the Administrator and Hospice Medical Director.
Experience Requirements
- A minimum of five years of nursing experience, with a focus on clinical care in oncology, hospice, geriatrics, or intensive care preferred.
- Active certification as a clinical Nurse Specialist or Nurse Practitioner in the relevant state.
- Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
- Valid driver's license and vehicle insurance, with access to reliable transportation.
- Current CPR certification is mandatory.
- No additional state-specific requirements.
- Palliative Care ARNP certification is desirable.
- Strong understanding of the interdisciplinary team approach in healthcare.
