Role Responsibilities
The Hospice Nurse Practitioner will provide expert consultation in palliative care, focusing on symptom management and supportive care tailored to the needs of hospice patients as referred by their physicians.
- Conduct face-to-face visits with qualifying hospice patients within designated timeframes, documenting clinical findings for the Hospice Medical Director.
- Collaborate with the Hospice Medical Director and primary physicians to recommend services and coordinate care.
- Develop and manage individualized care plans, documenting patient issues, goals, interventions, and responses to care.
- Educate and guide patients and families in self-care techniques when appropriate.
- Maintain precise clinical records regarding patient conditions and communicate any significant changes to the Administrator and Hospice Medical Director.
Experience Requirements
- A minimum of five years of nursing experience, with a preference for clinical expertise in oncology, hospice, geriatrics, or intensive care.
- Active certification as a clinical Nurse Specialist or Nurse Practitioner in the relevant state.
- Certification from a recognized national body such as ANCC, NBCHPN, or AANP.
- A valid driver's license and vehicle insurance, along with access to reliable transportation.
- Current CPR certification is mandatory.
- No additional state-specific requirements are necessary.
- Palliative Care ARNP certification is desirable.
- Strong understanding of the interdisciplinary team approach in patient care.
