North Suffolk Community Services Inc. is seeking a highly motivated and compassionate Community Outreach Specialist to join our team. As a Community Outreach Specialist, you will play a vital role in providing support and services to individuals with severe and persistent mental illness and/or substance use disorder.
Key Responsibilities- Engagement and Assessment: Engage with individuals and their families to assess their needs and develop personalized treatment plans.
- Community Support: Provide support and coaching to help individuals achieve symptom stabilization, self-management, and skill-building.
- Service Coordination: Coordinate with community agencies to provide services and support to individuals.
- Recreational Activities: Organize and lead individual and group social and recreational activities to promote socialization and community integration.
- Treatment Plan Implementation: Carry out assigned interventions and action steps of the treatment plan, providing direct support and rehabilitation services to individuals.
- Medication Administration: Participate in medication administration per MAP and DMH policies and protocol.
- Education: High School Diploma/GED equivalent required; Bachelor's Degree highly preferred.
- Experience: At least one year of experience working with individuals with severe and persistent mental illness and/or substance use disorder.
- Skills: MAP and CPR/First Aid certification required within five months of hire; Massachusetts Driver's License required.
- Paid Time Off – Two weeks' Vacation per year (three weeks after 2 years of service), 12 Sick Days per year, and 11 Paid Holidays per year.
