Outreach Specialist

North Suffolk Community Services Inc. is seeking a dedicated and compassionate Outreach Worker to join our Community Supports Program.
The successful candidate will provide intensive case management services to adults and children with mental health and substance abuse issues who are at risk in the community. Key responsibilities include:
- Establishing and maintaining relationships with clients, providing ongoing support and contact.
- Acting as a liaison between service providers, community, and clients, while conducting comprehensive initial assessments of need areas.
- Conducting risk assessments with each client and developing crisis plans.
- Assisting individuals in accessing medical appointments, outpatient mental health treatment, and other essential services.
- Supporting clients in navigating official correspondence related to insurance eligibility, social security, and other entitlement programs.
- Helping clients identify and access resources to support their recovery.
To be successful in this role, you will require:
- Education: A Bachelor's degree in a related field (behavioral health) or relevant work experience, or lived experience of homelessness, behavioral health conditions, and/or justice involvement.
- Experience: Previous outreach experience with populations with mental illness and/or substance abuse is strongly preferred.
- Skills: A valid Mass driver's license, reliable transportation, and bilingual ability (strongly encouraged to apply).
