Job Summary: We are seeking a highly motivated and compassionate Mental Health Outreach Specialist to join our team at North Suffolk Mental Health Association. As a key member of our outreach team, you will play a vital role in engaging with individuals who require support and guidance to achieve their goals and improve their overall well-being.

Key Responsibilities:

  • Engage with individuals in community settings to assess their needs and develop personalized treatment plans.
  • Collaborate with a multidisciplinary team to provide person-centered interventions and support services.
  • Assist individuals in finding safe and affordable housing, and provide support with daily living skills.
  • Administer medications as per MAP and DMH policies and protocols.

Requirements:

  • High School Diploma or equivalent required; Bachelor's Degree preferred.
  • At least one year of experience working with individuals with severe and persistent mental illness and/or substance use disorder.
  • MAP and CPR/First Aid certification required within five months of hire.
  • Massachusetts Driver's License required.
