Community Support Specialist

North Suffolk Community Services Inc. is seeking a skilled Community Support Specialist to join our team. As a key member of our outreach team, you will play a vital role in engaging with individuals, assessing their needs, and developing personalized treatment plans to support their mental health and well-being.
Key Responsibilities- Engage with individuals and families to understand their needs and goals, and develop tailored support plans to promote their health and wellness.
- Collaborate with a multidisciplinary team to provide comprehensive support services, including housing, risk mitigation, transportation, skill-building, and medication administration.
- Provide person-centered interventions and coaching to help individuals achieve symptom stabilization, self-management, and skill restoration.
- Conduct assessments and gather information about community agencies to inform service provision.
- Organize and lead individual and group social and recreational activities to promote socialization and community engagement.
- Carry out assigned interventions and action steps of the treatment plan, providing direct support and rehabilitation services to individuals and families.
- Collaborate with Housing Coordinators to support individuals in finding safe and affordable housing, including apartment hunting, finding roommates, and landlord negotiations.
- Participate in medication administration, adhering to MAP and DMH policies and protocols.
- Education: High School Diploma/GED equivalent required; Bachelor's Degree highly preferred.
- Experience: At least one year of experience working with individuals with severe and persistent mental illness and/or substance use disorder; bilingual Spanish and American Sign Language skills desired but not required.
- Skills: MAP and CPR/First Aid certification within five months of hire; Massachusetts Driver's License required.
- Paid Time Off – Two weeks' Vacation per year (three weeks after 2 years of service), 12 Sick Days per year, and 11 Paid Holidays per year.
