Job Summary

North Suffolk Community Services Inc. is seeking a skilled Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in the engagement, assessment, and treatment planning process for individuals served. You will work collaboratively with a dedicated team to address the needs and goals of our clients in both community and group living environments.

Key Responsibilities:

  • Engage with individuals and partner with them to improve their health and wellness, achieve symptom stabilization, and develop self-management skills.
  • Gather information about community agencies to provide support services.
  • Provide side-by-side support and coaching to help individuals socialize and develop skills.
  • Organize and lead individual and group social and recreational activities.
  • Implement interventions and action steps of the treatment plan through direct support and rehabilitation services.
  • Collaborate with Housing Coordinators to assist individuals in finding safe and affordable housing.
  • Participate in medication administration according to MAP and DMH policies and protocol.

Requirements:

  • Education: High School Diploma/GED equivalent required; Bachelor's Degree highly preferred.
  • Experience: At least one year of experience working with individuals with severe and persistent mental illness and/or substance use disorder.
  • Skills: MAP and CPR/First Aid certification required within five months of hire; Massachusetts Driver's License required.
