Clinical Nurse Specialist for Transplant Coordination

Position Overview: This is a Full-Time role for a Pre-Liver RN Transplant Coordinator. We are seeking a dedicated RN who values teamwork, flexibility, and a patient-centered approach to healthcare.

Key Responsibilities:

  • Demonstrate exceptional communication skills while managing extensive computer work, including proficiency in EPIC.
  • Bilingual candidates, particularly those fluent in Spanish, are highly preferred.
  • Work schedule is Monday through Friday, with on-call duties approximately every five to six weeks.

Qualifications:

  • Associate degree in Nursing is required; a Bachelor's degree in Nursing is preferred.
  • 3-4 years of experience in Medical/Surgical or Intensive Care settings.

Clinical Duties:

  • Collect and analyze relevant clinical data through a systematic and ongoing process.
  • Interpret tests and procedures according to established evidence-based standards of care.
  • Anticipate and address the expectations and needs of patients and their families to develop suitable care plans.
  • Maintain and apply competencies in line with unit expectations, synthesizing data relevant to organ-specific transplantation.
  • Utilize critical thinking skills to plan and prioritize nursing care effectively.
  • Identify and implement appropriate nursing interventions based on thorough assessments.
  • Coordinate interdisciplinary care plans and communicate effectively with patients, families, and transplant team members.
  • Continuously assess and revise care plans based on ongoing data and expected outcomes.
  • Document interventions and modifications to care plans accurately.
  • Adhere to policies, procedures, clinical pathways, and accepted nursing standards while delivering patient care.
  • Provide transplant-specific health education that encourages patient self-monitoring and promotes self-care.
  • Ensure compliance with transplant regulatory policies related to organ allocation and candidate listing.
  • Advocate for patients' medical and legal rights, promoting informed consent processes.
  • Address patient and family complaints effectively and report them to the Clinical Nurse Manager or designated personnel promptly.
  • Participate in scheduled staff meetings and mandatory educational sessions to maintain professional development.
